ROLE OF RED BLOOD CELL DISTRIBUTION WIDTH AS A PROGNOSTIC MARKER IN PATIENTS WITH SEVERE SEPSIS AND SEPTIC SHOCK.


Red Cell Distribution Width (RDW) is widely available to clinicians, because it is routinely reported as part of complete blood count. Several studies have reported that RDW is closely related to outcome in critically ill patients. Severe sepsis and septic shock are increasing in incidence and contributing significantly to mortality. Prediction of outcome for patients with sepsis using easily available and reliable marker may facilitate more aggressive interventions made at appropriate time. We studied this correlation and whether changes in Red cell distribution width reflects acute changes in disease progression.

Massive victory by AAP !

Massive victory by AAP ! Much needed reality check for BJP , which was taking people for granted and this is a tight slap for some of it's arrogant MPs.
This also shows the maturity people have attained as a voter.
However comparing this to "Modi wave" would be foolish , because people have voted against a "component" of BJP (intensified by highly embarrassing cameo by Kiran bedi showing classic example of opportunism) and voted for a man who promised DIRECT intervention,and who had shining trailer of 49 days to his credit.
BJP still remains favourite for the top job and it's AAP's coming 5 years in Delhi that will decide fate of both in future.
Working as a team is much more difficult than working individually (my personal experience) ,thats where Mr. Modi needs to pay more attention and that also may be the biggest obstacle in progress of AAP.
Finally congratulations to AAM Aadmi !!

India has glorious history of dedication and patriotism. We are lucky to have men like Shahid Bhagat singh and Mahatma Gandhi, borned in our country. The fundamental thing which differentiates these men from rest of the others is their true unselfish thinking…action for the cause, without expecting name, fame or money.

The era we are living today is the era, which demands selfishness. If you want to achieve remarkable success, you need to break rules, you need to have relations, you need to find shortcuts..which is possible if you are empowered with money….some may differ on this, they will claim that they have succeeded through honest hardwork…this is really appreciable..but just notice, I have used ‘remarkable’ success in that sentence…and I am sure if the succeeded persons are at some position now, they would have reached still higher if they were a part of transparent system ! No wonder we find lots of undeserving people at the positions where they are not expected to be !!

I think students are the direct victims of corruption, because this is the stage where the consequences have direct impact on the future functional population of the nation. This stage decides how the genuine talent will be used in development of the country..Or it will be wasted due to fatal corruption to prefer a rich nondeserving!!

So as you might have guessed, this is related to India against corruption! This is not new for us…at some or the other point we had faced consequences of it. And we were helpless; we could not do anything apart from cursing the system, only to face further consequences in the future. Remember this point “WE WERE HELPLESS, WE COULD NOT DO ANYTHING APART FROM CURSING THE SYSTEM” !!

The problem all these years was lack of such a figure to lead mass movement against unchecked corruption ! As I already said, it’s hard to have such unselfish men (like those in the past) to lead this most needed movement, even they are honest..It’s even harder that all of the population will believe their honesty and their motives…so there will always be significant lack of support. Corruption is prevailing, just because of lack of counter attack. It’s a vicious cycle..In which power of these corrupt people is growing and most important, it is transferable to their progeny, keeping in mind, the fact that a politician produces a politician son and rich father produces rich son…corrupt mentality is a gift to their progeny…and the resources are provided in the form of money and political power... look at the govt.’s lokpal…All the "MPs" will not be covered under it...Means what?? They are the only suckers.. MPs, in my area, which are like rodents... Just look at major scams in recent times..Can you imagine, these would not be covered by govt’s lokpal !!

Attempts have been made since 1980s to check abuse of political power !! Right to information act is classic example. Anna hazare was trying for it since 90s, and succeeded in bringing it to action. Why few people are not in favor of strong lokpal…?? At the age of 60-70, these people have not much time left to show their corruptive power, but they want to make sure that, their set up for their progeny should not be under the scrutiny. I won’t request you to support the movement, because I have explained you something..I know people are intelligent, and they can comprehend this issue…I am just clearing up the things. I have gone through both versions of lokpal…and I know even minor who had faced corruption at basic levels would support Anna’s version (as everybody is supporting).

Now there are two issues..First: few people think that, it would be an insult of parliament. How can few people make law outside the parliament??..Remember that point “WE WERE HELPLESS, WE COULD NOT DO ANYTHING APART FROM CURSING THE SYSTEM”.. Remember that even this version of lokpal was drafted only after Anna’s fast…that means they would have done nothing, if Anna wouldn’t have initiated the movement. It’s like, if someone is robbing our property or insulting our ladies ,then we should not do anything, because we are not police…is it like that?? This is no way an unlawful act…they are not making any law against society..they have majority on their side…if few people have problem because of this issue, then they should carry out legal procedure, accept the people’s lokpal and make the laws in parliament itself…so issue is only to bringing the people’s lokpal, they are not willing to show their power…they are least bothered about the way how it is brought…they simply want to bring it…

Second issue is, why should people trust Anna and his team ?? Giving full respect to people’s intelligence and their suspicious nature…I am not asking them to trust team Anna !! I am asking them to support for a cause ! Why the people are not able to see forward?? Just consider the end result…a strong lokpal will be in action because of their huge support…no matter who was leading the protest ! If they still has an issue with it, then it means that they have issue with the lokpal itself…in that case they should themselves go through the drafts..And I am sure, there will be no need to make their minds, they will be convinced with that !!

At the end, I am from Maharashtra, and have been witnessing Anna’s work since childhood...There’s no doubt about honesty of this man...Few people if have problem with this man, please consider the CAUSE only, its transparent and exactly the most needed thing...At least the team Anna has all the intelligent heads, who were class 1 officers..You can go through Anna’s work on Wikipedia..And if you think wiki is user edited, then check govt. website regarding history of right to information act !!
So I kindly request every citizen to support the movement (if not Anna) on august 16th, 2011…student’s union has vital role to play …youth of India should take this seriously…jai hind !!

Being a professor at medical college

In maharashtra, People who have done md medicine are paid Rs. 80,000 to 90,000 per month, if they work as assistant professor in any govt. medical college. They are allowed to practice outside also, through which they earn nearly 60 to 80 thousand, simply because most of the patients from general population get used to their names when they interact with the same in medical college.

What the asst. Professor does in medical college is more interesting !
After taking round in wards which takes just 15 to 20 mins or less, they sit in college canteen to enjoy tea for nearly 30 mins, another 30 mins to talk with class 3 ,class 4 workers of the hospital and local so called politicians...This would help to grow their private practice outside...Also by this they have covered nearly 1 and half hour of their OPD hours (which is in total of 3.5 hours and only two days a week)...Now after 1 pm onwards they get busy in their private practice and there are no evening rounds at medical college.

For their amusement they can arrange a half hour clinic or lecture twice a month, if they think so, And can enjoy playing jokes on the medical students, which the students themselves enjoy, surprisingly ! (may be due to all the frustration they face in such scenario)
Dont think that patients are kept waiting for treatment, there is huge army of doctors to treat them. Starting from Interns, H.O.s, diploma students, PG students, registars, lecturers ; so the Asst. Professor is not expected to worry about anything, he is left with the only job of signing some documents or torturing any student for not doing his duties and extending his/her posting. (ofcourse the quality of treatment is compromised)

More interestingly 'Professor and HOD' doesnt need to even visit the medical college for months ! And he is paid Rs. 1,20,000 per month for being on that post. (notable fact is, Interns are paid Rs. 2550 per month for round the clock duty)

People are earning Rs. 2 lakh and more per month through such a cool setup, and particularly some talented surgeons even more (3 to 4 lakhs). This is the truth and i have all the proofs.

To get such kind of job ,you only need to do MD medicine or MD in any clinical branch, and then Pass the required MPSC/UPSC exam. And here also you will be getting all the social rewards.
People why are you after radiodiagnosis , if such an alternative is available?

P.S.: Nothing personal with anyone ! And i know some exceptions are there, who do their job honestly. This applies to everyone who fits in the story.

This is really appreciable, vngmc yavatmal is leading from the front in the protest against govt. of maharashtra,for increasing the interns stipend.
The movement was initiated & intensified at our very own college. Thanks to Ankush Nawale, Gururaj and all the vngmcians, as well as all the interns across the state.
That was needed since long, but alas, we couldnt do it. 2004 batch had succeeded in raising it by little margin of Rs. 800, the response which was more likely an insult rather than sympathy. But they had no other choice than accepting it.
In a while, they and our college would be featuring in leading newspapers and why not, they deserve it.
Interns all over the state would get benefits from this, including the interns who, buy time (by avoiding postings) to prepare for pg entrance, for them this money would be very useful to buy that precious time. That seems practical enough, even if the same people support INDIA AGAINST CORRUPTION, ultimately they have to be practical and thats what is the need of time. Opinions may vary. Generally opinions came from the situation (of the person) and not by person itself , so its not good to measure anyone in terms of right or wrong. People have their own choice, but the bottom line is, since i have gone through the hectic internship in this college (svngmc yavatmal), let me dare to say, No one deserve better than the interns here, to initiate such a movement and to get such a stipend of Rs. 10000+ per month (if it happens)

NEGATIVE EVOLUTION: The adaptation for dependence

Currently I am posted in Obs. & Gynec., and shocked with the huge no. of complications occuring in the process of child birth, which is considered ,one of the natural process.

Numerous medical interventions in the natural process of childbirth, since considerably long time; have completely dominated its naturalness. Its a good example of NEGATIVE EVOLUTION(?).

Now those changes which wont allow to carry the process on its own, (like increasing no. of episiotomies, slower physiological responses from mothers) have well established in the gene pool of the population (particularly those from upper classes) .

No wonder, now not a single case can be allowed to go without medical aids.

So the word EVOLUTION was defined like "...Which SOMETIMES leads to better life of organism..." that means SOMETIMES it may not. So its only a process of gradual adaptation,...Even adaptation for dependence.

This is the big, very very very big concept !! Just imagine when you say my mind...Then actually what do u mean??


For others, you are heavily represented by your physical body ! But inside u, when u think of yourself, you are represented by the impulse of "ME"ness, which is operating through thought process !


But still, when u say "MY" mind, It proves that, there is something over your mind...Which operates you..& thats why u can independently think of "YOUR MIND" & "BODY" separately...& interesting thing is that, it is out of your control.

I concluded that, Death is exactly when...When u cant just think of your body & your mind. That means the third mysterious faculty has been switched off !

So the weakest thing of your mind is that, its a slave of something you are aware of, whom you call "I", but exactly which is not in your control ! Your job is to free it !!

I take this opportunity, to advice you, to go through the movie MATRIX...the greatest construction of human civilization!!

My letter to MUHS Nashik and MCI

The concerned officer.

Respected Sir/Madam,

Myself, Vaibhav Yawalkar, a student of Shri. Vasantrao Naik Govt. medical college, Yavatmal, affiliated by Maharashtra University of Health sciences (MUHS) Nashik (Maharashtra) , have just finished my MBBS, and now will be undergoing internship in the same college.


Our university avails the facility for transfer of internship after completion of concerned course, to various medical colleges across the country.

The rules regarding transfer of internship are available on university website and can be found here-

According to these rules ,transfer of internship will be allowed on merit basis (based on marks scored by the candidate in the final year MBBS exam) and also to special cases (such as, on medical ground) as said by the university.


But there is no information or the information is unclear, regarding MAXIMUM number of candidates that will be transferred from particular college to the other, which also decides the MINIMUM number of candidates that will remain in the parent college and is more important issue. Let me clarify this.


As per new rules the number of candidates to be transferred is 15% of total candidates APPEARED for final year MBBS exam during that academical year in the same college.


At the first place, I personally feel that, the number of transfers should be 15% of total candidates PASSED in final year MBBS exam in the same college and should not be calculated on the basis of number of students APPEARED for the exam. This will maintain the number of interns in that college to serve the patients, which is (should be) our foremost aim. And it will also reduce extra burden on the interns that would have been there if the number of transferred candidates were large.


Second thing is that, whether the 15% transfer limit is inclusive of special cases or they are done separately outside the 15% quota? If it is done separately, then I kindly request you, to finalize the 15% transfers limit applicable as a total number of candidates that will be transferred inclusive of all categories mentioned.
After all maintaining the minimum number of interns in any medical college should be given highest importance rather than giving it to individual medical student.


Third thing, I would like to suggest that , transfer proposals under special cases category, such as on medical grounds, should be examined carefully and by on-spot medical examination of the candidate or the relative of the candidate and not merely by considering the submitted medical documents to prevent abuse of this facility.


Fourth thing is that, according to the notification MUHS/EO/1403/2008 dated 08/04/2008 (can be found here : http://www.muhsnashik.com/academic/Regarding_Internship_Transfer_290308.htm)

point "G" states that,
" In ADDITION to above all categories, the Hon'ble vice chancellor will have power of granting internship transfer to DESERVING candidate in special circumstances "


kindly clarify the following queries regarding above statement:

1) What exactly the word, "DESERVING" means, does it mean that the candidate still has to satisfy merit criteria or special cases criteria ? If NO, then what are the criterias so that the Hon'ble vice chancellor considers that particular case?


2) Whether these transfers are outside 15% quota or part of it? And if outside, then what is the limit for number of transfers under this category?


I kindly request you to consider my suggestions and take immediate action regarding the same, and also clear the asked queries.

Misleading results...Need to search new genuine ways to test one's caliber

Today is a very special day for me, as I cleared final mbbs examination and completd my UG, not only because of this, but it is also the worst result of my life. I was expecting something bad but definitely not the worst.
So I thought at this point some questions should be answered, don’t think that these questions are the result of frustration, coz I never felt proud of my scores in various exams (despite some of them being remarkable) or was disappointed with the bad ones at any instance, and I believe the old saying, “Numbers are like ******, what they reveal is suggestive , but what they hide is more interesting”


The only reason for this is my concern over the rumors (?) about the government’s decision to shut the PG entrance examinations in the state, and forming the final year UG scores as the new criteria for admissions. At the very first place, I have never been able to understand the logic behind, how these people at important posts can even think of such nonsense things.


Now there are plenty of factors which finalize how the student will perform in these traditional exam systems..No doubt his own talent is the chief one..But don’t you think , there are also some very cheap factors..Mostly helping the false talent and underestimating the genuine one..And why these cheap factors can affect the final outcome?..simply because of cut-throat competition..I will talk about our UG exams..


Some students can memorize every topic quickly and are able to write it down exactly as they read…of course it’s a talent…not everyone can do this, but do they learn anything from it to apply the same somewhere…if the answer is NO then what was the purpose to read it? clearly it was ,to score some good marks in the exams….which is a very fatal tendency…often I see many students who have studied basics of math, physics, chemistry and electronics in their schools and colleges, don’t know anything and fail to apply their knowledge(?) in day to day life..this group also contains the candidates who have scored 100% marks in these subjects…everyone knows how these exams are taken, same questions, same pattern…even the marks on individual chapters are decided..you have to write only with the method that is given in the textbook, you are not allowed to think differently and to devise your own method, otherwise no marks are credited…also you have to write systematically with good handwriting, well labeled diagrams, margins, all steps..and if you miss to summarize your final answer in a sentence at the end, then your one mark will be deducted …how funny!!! Don’t these people know that, true intelligent persons are not that disciplined ?? (Einstein, Newton….list is long) coz they simply don’t give importance to useless facts…and for this one separate subject titled “DISCIPLINES” should be included in the syllabus and there we can exercise all these things…at no place such restrictions should cost any talented person..


Viva….this is better option for theory examination, but unfortunately there are also OTHER factors that can come to play depending on the examiner…for example some examiners immediately get impressed with nice selection of words, fluency, no matter how much irrelevant he/SHE is speaking..some examiners never ask anything related to the topic…some of them will ask a very hard question right at the start and if unanswered…they assume that he (should I write SHE??) knows nothing…this also happens when you forget to read some topic accidently…and you should guess what examiners want from you (this is tough one)..coz if you ask him “is this what you wanted?…” or “exactly what do you mean?....or “with what reference…?” …then he can fail you right on the spot…


MCQ’s have excluded the dictator’s bias towards particular group while dictating theory papers…coz we often see a group of students belonging to particular batch gets higher marks than the other group of same class…this magic is due to different dictators …with different moods and different mindsets…and the degree of STUDENT FRIENDLY attitude…since while checking MCQ’s examiners are not allowed to think (but the students are…that is in the examination hall…its easier to cheat when it comes to MCQ’S…he..he..)


Well jokes apart…but MCQ’s (not objective..but those which need, processing the information and using the concepts) is the best way to test the talent…and I request you all people to unite to save examinations based on this pattern and to prevent politics and the money from being deciders of the fate of talent..

While thinking about the origin of life, about our purpose for being here, about our placement in the universe and how we are spared from all the threats from space...i used to finish my every thinking time slot, by arriving at the conclusion that there is something in the universe , the ultimate power that is controlling everything in this world. some call it as GOD, some believe it to be THE NATURE, some call it as THE ILLUMINATI...but THE ONE exists thats for sure. I have been thinking this since long time but when i watched the master creation THE MATRIX by wachowski brothers, my senses were shattered and i don't know why, after that i started to think that we can reveal this power by solving the unsolved mysteries..because i think these are the portals which represent the faults in this CONTROL.

so coming to the topic, can you divide anything by zero ?
As established, many will answer it as infinite...but certainly thats not the correct answer, as long as we know actual difference between the terms INFINITY and UNDEFINED, we should take some time before we arrive to some conclusion.

If asked, what is infinity ? then the most probable and proper answer would be, that which never ends or something that whose extremes we can not measure. for example the series 1,2,3,4,5,...... has no end, that is the series is infinite. But every SO CALLED END VALUE of the series will be the pure number, that is even if we are calling it as infinite, still we have a measurable quantity, each time we proceed to next.

And what is UNDEFINED?
Now consider 1 divided by zero, just for easy justification...for some moment keep aside arithmetic means and analyse what is the division? . it is distribution of some DEFINITE quantity in a DEFINITE no. of DEFINITE equal parts. this means every aspect of the process should be defined and it should purely exist.
suppose you have to distribute Rs. 10 among 5 people, then you do 10/5 and get answer as Rs. 2 for every person. Now consider you have to distribute the same definite amount among zero people? in this process you don't have the DEFINED no. of people among whom the quantity is to be distributed. so the process is UNDEFINED and the answer is also UNDEFINED.

why the answer is not INFINITY?
simply because if you are saying this answer then you have to distribute INFINITE quantity among no one, and importantly OUT OF A FINITE DEFINED QUANTITY, which makes no sense, HOW ONE CAN GET INFINITE EQUAL PIECES FROM A FINITE QUANTITY ??

if this is taking more time to understand, lets approach by other aspect. in 10/2=5, you have to add the no. 2 , 5 times to get 10. that is 10=2+2+2+2+2+(REMAINDER if any ,when the division is not complete). so think about 10/0... you have to add "0", that no. of times which will give the result 10. Offcource the answer is not infinite, even if you are adding "0" infinite no. of times ,you will never reach to 10, but the result will only be ZERO. the pure zero, the pure TRUTH.
so thats why the answer is UNDEFINED.

So in one sentence i say that an UNDEFINED is always an UNDEFINED, while INFINITY at any of its instance is always measurable.

What i derived is "1/0= 0"...."anything divided by zero is zero" how ??
It simply means you are distributing a finite quantity among no one, so you have to take nothing out of that quantity, so you have to take zero out of it , hence the answer is zero and whole quantity remains as remainder.
so,
11 / 2 = 5 x 2 + 1
similarly,
A / 0 = 0 x 0 + A ( many will comment that you can write any result, even infinite in this format, as whatever multiplied by zero will become zero, so they may say that

"A / 0 = {.....-3,-2,-1,0,1,2,3,...} x 0 + A" (multiple answers)
but then they should recall the process of distribution as described above, keeping aside algebra, so finally answer will only be the ABSOLUTE ZERO.
